چکیده
A 63-year-old woman developed conjunctival hyperemia and blurry vision. She was diagnosed with uveitis. A chest computed tomography (CT) scan detected mediastinal and hilar lymphadenopathy (Picture 1A) and the diffuse small nodules that were slightly predominant in the left lower lobe (Picture 1B). A scalene lymph node biopsy demonstrated non-caseating granulomas (Picture 2A, ×200). She was diagnosed with sarcoidosis. After six months, a chest CT scan revealed the enlargement and increased numbers of the nodules in the left lower lobe (Picture 1C). A transbronchial lung biopsy revealed encapsulated yeast in the alveolar space (Picture 2B, Grocott staining, ×200 and ×1,000, arrow; Picture 2C, Periodic acid-Schiff staining, ×200 and × 1,000, arrows). The patient’s serum was positive for Cryptococcus neoformans antigen. She was also diagnosed with pulmonary cryptococcosis. Fluconazole treatment resulted in the improvement of her chest images. The complications of pulmonary cryptococcosis in untreated patients with sarcoi-

Disseminated cryptococcosis with pulmonary and marrow involvement mimicking radiological features of malignancy.
The most commonly involved sites of cryptococcosis are the lungs and the central nervous system. Cryptococcal osteomyelitis is a rare complication of disseminated cryptococcosis, and the vertebraes are the most common site of this infection. The most common underlying disease is sarcoidosis, followed by tuberculosis and previous steroid therapy. Conservative treatment alone or treatment with a ...
متن کاملRare Association of Severe Cryptococcal and Tuberculosis in Central Nervous System in a case of Sarcoidosis
Sarcoidosis is a multisystem noncaseating granulomatous disease with a propensity for lung, eye, and skin which recently have been proposed that mycobacterium tuberculosis may contribute in its pathogenesis, and rarely involves central nervous system (CNS). Despite CD4+ lymphocytopenia, sarcoidosis by itself does not increase risk of opportunistic infections other than cryptococcosis. Nonethele...
متن کاملCryptococcosis in a patient with HIV infection
Background Cryptococcus neoformans is encapsulated yeast with worldwide distribution, which may cause a self-limited pulmonary infection or disseminate (especially to the meninges, but sometimes to the skin, bones, viscera, or other sites). The infection is acquired by inhalation of contaminated soil. Cryptococcosis is a defining opportunistic infection for AIDS; also at increased risk for infe...

متن کاملClinical Features of Pulmonary Sarcoidosis Complicated by Lung Cancer
Objective For lung cancer complicated with sarcoidosis, there are no exact features that indicate whether lymphadenopathy is metastatic. This makes the validity of surgery uncertain for clinicians. The aim of this study was to clarify the clinical features of pulmonary sarcoidosis complicated by lung cancer, especially from the viewpoint of evaluating lymphadenopathy. Methods We retrospectively...
